Ourmultidisciplinary teams offer culturally-sensitive, end-of-life andpalliative care services to patients and their families at home orin assisted living communities, skilled nursing facilities andhospitals. Working on-site at amajor medical center, the Hospice Care Planner is responsible forevaluating and establishing a plan of care for patients referredfor Hospice services. The Hospice Care Planner will facilitateadmission of referrals upon discharge from the hospital to theHospice program that meets the needs of the patient and familymembers. As a key member of the business development and marketingteam, the Hospice Planner develops and maintains positiverelationships with physicians, social workers, case managers anddischarge planners within the medical center.
