Ourmultidisciplinary teams offer culturally-sensitive, end-of-life andpalliative care services to patients and their families at home orin assisted living communities, skilled nursing facilities andhospitals. In this field basedposition, you will visit MJHS Hospice and Palliative Care patientsto perform both physical and psychosocial assessments andelectronically document visit occurences. Patient visits will bemade in both community and facility settings. As the Hospice FieldCase Manager, you will develop and implement nursing plans of careand make recommendations for care based on patients' needs. Asneeded, you will contact the patient's physician to request writtenorders to facilitate plan of care changes. In addition, you willalso supervise, train and evaluate home health aides and homemakersworking with our hospice patients.
