Theprofessional in this position will act as a patient advocate infacilitating, coordinating and communicating the transition ofpatients to next level of care in accordance with applicable law,regulations and agency policies and procedures. You will work in acollaborative role with MJHC and select inpatient facilities toevaluate inpatients referred for transitional planning and/or homehealth services.
