Ourmultidisciplinary teams offer culturally-sensitive, end-of-life andpalliative care services to patients and their families at home orin assisted living communities, skilled nursing facilities andhospitals. Reporting to the RN Team Manager, this position willtriage all calls for Hospice patients and provide direct nursingcare to patients when a home visit is necessary. Will develop andupdate the plan of care as needed requesting written orders forchanges from the physician. Will document all nursing care providedinto computer and will update status of patient as needed. Willcommunicate all changes to patients condition to primary RN casemanager. This Weekend Fee for Service position has 2 shiftsavailable!
