Inthis role, you will assume the ongoing, primary responsibility tocoordinate, implement, continually evaluate the home care needs aswell as providing direct patient care of your patients. You willalso review patient insurance information, coordinate activitiesinvolved in each individual care plan and make sure that pertinentfindings are shared with family and professional caregivers. Yourexcellent communication, documentation and time managementcapabilities will be essential to your success.
