Ourmultidisciplinary teams offer culturally-sensitive, end-of-life andpalliative care services to patients and their families at home orin assisted living communities, skilled nursing facilities andhospitals. In this field positionyou will perform initial physical and psychosocial assessments ofpotential hospice patients in order to develop individualized plansof care, goals, and make recommendations based on each patient'sneeds. Assessments may take place in private homes, hospitals ornursing facilities. As a Hospice Field Admission Nurse (RN), youwill admit eligible patients onto the Hospice program following theappropriate standards of practice established by the NationalHospice and Palliative Care Organization (NHPCO). In this keyposition, you will also engage in community outreach to physiciansand other medical providers to share information regarding the MJHSHospice program, and gather feedback to effectively informperformance improvement initiatives. The Admissions RN for thisposition will be conducting Hospice Admissions assessments in Bronx& Manhattan. The schedule for this position will be 11am-7pm.Please note this position also rotates one weekend per month and2-3 holidays per year.
