Do you have a "Hospice Heart"? If your answer is yes,then you are just the nurse we are looking for. When days cannot beadded to a life, we add life to remaining days. Ourmultidisciplinary teams offer culturally-sensitive, end-of-life andpalliative care services to patients and their families at home orin assisted living communities, skilled nursing facilities andhospitals. In this field based position, you will visit MJHSHospice and Palliative Care patients to perform both physical andpsychosocial assessments and electronically document visitoccurences. Patient visits will be made in both community andfacility settings. As the Hospice Field Case Manager, you willdevelop and implement nursing plans of care and makerecommendations for care based on patients' needs. As needed, youwill contact the patient's physician to request written orders tofacilitate plan of care changes. In addition, you will alsosupervise, train and evaluate home health aides and homemakersworking with our hospice patients. Fee for Service - Shifts basedon your availability and location preference! Shifts availableMonday through Sunday: 9am - 5pm
