[We Care from theHeart](https://vimeo.com/370935417) from [AmedisysVideo](https://vimeo.com/amedisys) on [Vimeo](https://vimeo.com). *What You Will Do: * As a hospice case manager for Amedisys, youwill have a special opportunity to provide, coordinate and directthe provision of hospice nursing and palliative care to terminallyill patients in their home. You will closely collaborate with theattending physician, patients and their families, and other membersof the patient care team to provide and maintain continuity ofpatient care to achieve excellent pain and symptom management andhigh quality end-of-life care for the patient. * Assess andreassess all physical, environmental and emotional factors todetermine hospice needs for patients in the home setting * Works incooperation with the family/caregiver and IDT to meet the emotionalneeds of the patient/family/caregiver. * Utilize a combination ofagency resources and nationally recognized standards of practice toachieve excellent pain and symptom management and high qualityend-of-life care. * Coordinate the total Plan of Care and maintainscontinuity of patient care by liaising with appropriate staff andthe physician. * Actively participates in Interdisciplinary Team(IDT) conferences. * Provide supervision and evaluation of aidesand nurses. * Attend funerals/calling hours when appropriate asassigned by the bereavement coordinator. * Qualifications* *Required: * * Current and unencumbered license to practice as aRegistered Nurse specific to that state the employee is assigned towork by the Company. * Minimum of one (1) year of experience as aregistered nurse * Current CPR certification * Valid Driver'sLicense and reliable transportation and agency required liabilityinsurance to complete home visits * Preferred: * * Bachelor ofScience in Nursing (BSN) degree * Previous home health or hospiceexperience * Certified Hospice and Palliative Nurse (CHPN) Amedisysis an equal opportunity employer.
