Essential Functions:1. Provides personal care services per state/federal rules, regulations, and/or standards.2. Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning and doing household chores.3. Plans and prepares nutritious meals, including shopping and assisting with feeding, as assigned.4. Performs homemaking and other environmental services as assigned.5. Observes, reports and documents any changes in patient's status. If unable to perform a certain task, reports to the primary nurse.6. Understands basic elements of body functioning and reports changes in patient's body functions as indicated.7. Recognizes emergency situations and implements appropriate emergency procedures per care center policy.8. Follows care center procedures regarding infection control, handling of hazardous wastes, and safety measures.9. Accurately documents care provided and submits notes and other required documents per care center policy.10. Follows specific written instructions for personal care prepared by a registered nurse in accordance with the Plan of Care. Written instructions are to be received prior to the delivery of services.11. Assists patient with prescribed exercises as instructed per nurse or therapist.12. Provides respite for patient's family/caregiver when on-site, as appropriate.13. Performs and records simple urine tests for sugar, acetane, and albumin.14. Advocates and ensures patient participation in care planning and goals of care. * Supervises caseload including LPN's and Home Health Aides. Facilitates care coordination with PRN staff when care is provided for caseload. * Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions and guidelines, including OASIS assessments, SOC/Admissions, Recertifications, ABNs, subsequent visits, physician orders, care coordination etc. * Helps to achieve and maintain continuity of patient care by communicating information (for example, changes in plan of care, new orders, lab results, etc.) timely, effectively and systematically with all team members providing care for the patient including PT, OT, ST, MSW, paraprofessionals, physicians, family members and community resources. Facilitates interdisciplinary communication and follow up through care coordination on assigned caseload for duration that patients are on service. * Participates in Team/Case Conferences. Leads discussion on multidisciplinary cases as appropriate. 12. Maintains ongoing communication with clinical manager regarding progress of patients for assigned caseload. * Remains knowledgeable of organization's quality improvement programs, participates in agency performance improvement initiatives and incorporates into patient care planning as appropriate. * Demonstrates competency in skills for required patient care.
