We arecurrently seeking a Skilled Nursing Facility Physician responsiblefor the delivery of age and specialty appropriate medical carethrough the process of assessment; evaluation; diagnosing;recognition and prevention of adverse events; treatment; andfollow-up. Performs responsibilities in four areas of practice -clinician; teacher; leadership; and professional development.Collaborates with colleagues and other professional disciplines toensure safe; effective// efficient and value added patient centeredcare; and the achievement of desired patient outcomes. Continuouslymonitors quality patient care through excellence in customerservice and performance improvement that demonstrate positiveoutcomes in patient care. Provides leadership in the LPG practicelocation and throughout Lee Health. Possesses excellentcommunication skills with patient// family// caregivers//multidisciplinary team members// medical staff members// and allemployees. Keeps quality and safety in the forefront in thedelivery of patient care. Participates/supports the integratedcontinuum of care of patients within LH/LPG. We have access toleading edge equipment and technology connectivity with hospitalsand outpatient center through EPIC. This is a team oriented groupthat looks to provide high quality medicine while supporting theneeds of their patient and co-workers Opportunity for leadershipskillset- Liaison in the design and implementation of approvedstrategies and tactics to improve and streamline patient carewithin the adult specialty division. Interest in leading physiciansin a dyad model partnership with an operational system director.Interested in leading change, advancing patient care through theuse of digital tools, and acting as a disruptor to traditionalmodels of care delivery.
