Overview Job Summary: The Primary Case Manager RN inHospice manages and provides care to patients andfamily/caregivers. Provides nursing care and clinical casemanagement, manages resources and coordinated services with hospiceinterdisciplinary team.Completes comprehensive assessment/physicalassessment, modifies plan of care in conjunction withpatient/caregiver and hospice interdisciplinary team, providesinterventions and support to these patients and families to managesymptoms, support quality of life experience.. The Primary RNprovides the patient's nursing care and communicates coordinateswith full IDG team to support patient's and families carethroughout Hospice experience. This nurse demonstrates sensitivityto needs of culturally diverse populations by providing appropriatesound judgement in a professional manner in order to coordinate andsupervise patient care. The nurse assist patient and family withtransitions through end of life and understanding of hospicepractice and philosophy. Required Education: Diploma in Nursing,Associate's Degree in Nursing, or Bachelor's Degree in NursingRequired License: Must have and maintain current licensure asregistered nurse with the North Carolina Board of Nursing. Compactlicense may apply, licensee should confirm with NCBON; BCLS. ValidDriver's License.
