How will this role have an impact? Under thesupervision of the Member Engagement Manager, contacts members ofvarious Medicare and/or Medicaid health plan providers by phone tooffer, explain and schedule a free in-home healthcare riskevaluation. Subsequently assigns and schedules contracted providerfor each confirmed evaluation appointment. Adjusts and resetsappointments and schedules as may be required. Works diligently tomeet department goals for productivity and appointments scheduled.This role will report to our Member Engagement Manager. What willyou do? * Handles outbound calls placed by the telephony system toMedicare and/or Medicaid health plan members to offer, explain andschedule a free in-home health risk evaluation. * Follows theapproved script when discussing health risk evaluations withpotential member; ensure member understands and is comfortable withthe terms of the evaluation; answer * Presents a positive andprofessional face to clients, health plan members and workforce *Leverages Signify systems to schedule providers to conduct in-homemember health risk evaluations or other mutual agreed upon place;schedule providers presented by the system within the assignedgeographic area with appropriate time and distance between *Adjusts and reschedules appointments with both members andcontracted providers if requested; ensure cancellations andreschedules are recorded accurately and therefor communicated tothe appropriate parties internally. * Returns voice mail messagesfrom members, works manual calling list as required. * Seeks tosupport requested overtime to meet unexpected high volume
