2+ years of recent cardiology experience strongly preferred.

Summary: The Nurse Navigators are critical members of the team supporting patients with Chronic Heart Failure. A crucial part of their role is to triage patients with new or urgent issues, hoping to avoid emergency room visits whenever possible. In addition, complex care coordination is frequently required for many of our patients who live outside the Denver Metro area. Our CHF Nurse Navigators will split their time in clinic with the CHF multidisciplinary team, educating patients and ensuring a smooth flow of patients, along with out of clinic care coordination and triage of patients. Serves as a patient advocate, liaison, and advisor/educator to assist patients with navigating the continuum of care.

Work Schedule: Full Time Days

Location: Anschutz campus

Responsibilities:
Rounds on/reviews assigned patients regularly and evaluates patient progress with plan of care. Communicates plan of care to patient and family and solicits concerns, questions, and issues for resolution.

Provides emotional support, counseling, clinical education and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.

Serves as a liaison between patient and clinical staff, administration, physicians, managed care companies, and community/external resources to coordinate/maximize resources and identify/resolve barriers to the plan of care.

Assists patients and families with resolving financial, psycho-social, functional, and administrative issues by advising of options and referring to appropriate resources. Intervenes as appropriate to advocate for patient and family.

Withinscope of job, requires critical thinking skills, decisive judgement and theability to work with minimal supervision. Must be able to work in afast-paced environment and take appropriate action.
